Office Policies

CANCELLATION/ RESCHEDULE/ NO SHOW/LATE POLICY

Appointment times at Fuse Medspa & Wellness are reserved exclusively for each patient and require dedicated provider time, clinical preparation, and operational resources. Late cancellations, rescheduling, and missed appointments result in unrecoverable financial loss.

By scheduling an appointment, signing or electronically accepting this agreement, you accept full financial responsibility as outlined below.

Standard Appointment Policy (Under 90 Minutes)

  • A minimum of 24 hours’ notice is required to cancel or reschedule.
  • 23 hours and 59 minutes or less prior to the scheduled appointment:
  •  $100 cancellation/reschedule fee

Extended Appointment Policy (90+ Minutes or Designated Procedures)

Applies to:

  • Aveli Cellulite Treatment
  • iLaser Skin Treatment
  • PDO Thread Lift
  • Any appointment greater than 90 minutes

Fees for Extended Appointments

  • 47 hours and 59 minutes or less prior to appointment:
  •  $250 fee
  • 23 hours and 59 minutes or less prior to appointment:
  •  $500 fee

No-Show & Late Arrival Policy

A No-Show is defined as:

  • Failure to attend a scheduled appointment, OR
  • Arrival:
    • More than 5 minutes late for 15-minute appointments
    • More than 15 minutes late for 30-minute or longer appointments

No-Show Charges

  • 100% of the quoted service price will be charged
  • This reflects reserved provider time and lost business opportunity.

Deposits

  • Deposits are non-refundable
  • Applied toward the service if completed
  • Forfeited for late cancellations, reschedules, or no-shows
  • Additional charges may be applied to meet the full fee or service price

Card on File Authorization

By signing or electronically accepting this agreement, I authorize Fuse Medspa & Wellness to:

  • Store my payment method securely via Stripe and integrated systems
  • Charge my card for:
    • Cancellation fees
    • Rescheduling fees
    • No-show charges (including up to 100% of service price)
    • Any outstanding balances

Advance Financial Consent

I understand and agree that:

  • Charges may be processed without additional notice if I violate this policy
  • Charges apply to reserved appointment time, not solely services rendered
  • This agreement constitutes prior authorization for charges

Chargeback & Dispute Acknowledgment

I agree that:

  • I will not dispute valid charges that comply with this agreement
  • This signed/electronically accepted document serves as binding authorization and evidence of consent
  • I understand this policy is enforceable under card network rules when properly documented

Failed Payment & Collections

If my payment method cannot be processed:

  • I remain responsible for the balance
  • Fuse Medspa & Wellness may pursue payment to the fullest extent permitted by law, including:
    • Rebilling attempts
    • Collection agencies
    • Legal action

I agree to pay any applicable collection or legal costs where permitted.

Electronic Signature & Aesthetic Record Consent

I acknowledge and agree that:

  • This agreement may be executed via:
    • Aesthetic Record intake forms
    • Online booking system
    • SMS or email confirmations
  • My action of:
    • Signing digitally
    • Checking a required consent box
    • Booking an appointment

constitutes a legally binding electronic signature

  • My acceptance is stored with:
    • Date and time stamp
    • Patient record
    • Appointment details

and may be used as evidence in billing disputes

Liquidated Damages

I agree that all fees represent a reasonable estimate of damages, including:

  • Lost provider time
  • Inability to rebook
  • Staffing and operational costs

These charges are not penalties.

Repeat Violations

Fuse Medspa & Wellness reserves the right to:

  • Require prepayment
  • Restrict booking privileges
  • Refuse future services

Acknowledgment

I confirm that:

  • I have read and understand this agreement
  • I accept full financial responsibility
  • I authorize charges as outlined
  • I understand this policy is strictly enforced

Payment & Refunds/Returns

Payment is due at the time of service. No refunds on services rendered

Retail products (non prescription) may be returned within 30 days of purchase for refund or account credit or exchange

By law there are no refunds on ANY Prescription medication

We do not accept any insurance

The use of a HSA or FSA card is at your own risk and we are not responsible if your claim for payment is denied.

Behavior

We will always treat you with respect and kindness, and we expect the same in return. Any aggressive or threatening behavior will not be tolerated and you will be immediately dismissed from the practice. If you have any problems or concerns with a service or product purchased from us, we are more than happy to do our absolute best to make you satisfied, but aggressive behavior will not be tolerated.

Right Of Refusal

We have the right to refuse treatment that we deem inappropriate.
